Breast Enlargement

Technically, the breast enlargement is a fairly simple procedure. In addition to a skill for precision, the operating surgeon must have a sense of aesthetics in order to achieve good results. Preparation of the implant cavity must be carried out carefully under hemostasis. If this is done well, it is often unnecessary to place a drainage tube. The size of the implant mostly depends on what the patient desires; with saggy breasts, however, it may be necessary to select a larger implant or to combine the breast enlargement procedure with a breast tuck (also see Breast Tuck). Silicon gel is the recommended implant material. Today’s breast implants are state-of-the-art, high-quality, leak proof, and rarely cause capsule contractures, which can be treated if do they occur. Silicon breast implants also feel the most natural, so the new breast not only has a natural look, but also feels natural to touch. In April, 2004, the first silicon breast implant manufacturer began warranting a life-long guarantee on its silicon implants, which is evidence of the excellent quality of the implant shell and filling. Because guaranteed first-class implants are the only kind used for breast enlargements in Dr. Wolter’s practice, today’s patient can feel confident about the breast implants she receives.


Surgeons around the world agree that the positioning of the implant should be chosen to suit to the given circumstances. If there is little breast tissue, the implant is placed partially, or, in rare cases, completely under the muscle. If, however, there is enough tissue and if the breast is saggy, the implant is placed on top of the muscle. The correct choice of implant size and shape is also important. It is the surgeon’s task to identify the patient's wishes in the process of thorough examination and consultation on options and risks, and to accommodate him or her adequately. When each technique is correctly performed under the right circumstances and combined with the correct implant, breast augmentation results in both a natural look and feel.


The best way to access the breast for an enlargement is to make an incision along the crease beneath it or along the lower edge of the nipple areola. Both incision methods cause a limited amount of scarring that is later hardly noticeable. The advantage of the incision along the crease beneath the breast is that it leaves the gland tissue undamaged. Also, breast cancer screenings, which are essential and should be carried out on a regular basis, are hardly hindered by scarred tissue. Consequently, this type of incision is performed in Germany in 80% of breast operations. The third option, an incision made inside the armpit, is least common and makes up approx. 7% of breast operations, as it carries the largest rate of complications and is therefore not recommended. The procedure is carried out under general anaesthesia. Local anaesthetics that put the patient into a light sleep are only used in exceptional cases. It is usually necessary to remain in the hospital for one day, and, principally, it can even be carried out as an outpatient procedure. The solid tape bandaging applied directly after surgery is removed after about four days, and eight to ten days following the operation, the stitches are removed. The patient is to refrain from sports activities during the first four weeks following surgery and must wear a well supporting bra day and night. A bra should also be worn during sports activities for two further months.


Complications such as infections, secondary bleeding and wandering implants or capsule contractures with or without rippling and which require repeated surgery are rare. Patients are given thorough information on the probability and commonness of such complications in a consultation with Dr. Wolter. These risks are greatly reduced by the modern operation techniques applied by Dr. Wolter.


In general, complications occur less frequently with experienced surgeons like Dr. Wolter and the results are not only aesthetically superior, but safer as well.
